A Physician's Guide to Personal Health Program


This program provides background information and tools to help physicians support their personal efforts to live a healthier lifestyle and to change four key health behaviors: diet, physical activity, alcohol consumption and tobacco use. The toolkit describes the importance physicians play in fostering lifestyle change as role models to their patients, and explains how physicians can implement strategies to assess their own readiness to change and how to implement those changes. This brief toolkit is written for physicians by physicians, and yet the tools can apply to any busy professional. The toolkit provides a screening milestone document, action plans and progress tracking calendars.
