• Dabigatran etexilate is an oral anticoagulant that acts as a direct, competitive thrombin inhibitor. It does not require routine monitoring and has been approved by the FDA for the prevention of stroke and systemic embolism in patients with atrial fibrillation.
• Note, however, that no effective reversal agent for dabigatran in the event of catastrophic hemorrhage has been identified.
• This case report indicates an example of catastrophic hemorrhage after mild traumatic brain injury, and the authors suggest measuring thrombin time and instituting the early use of dialysis in conjunction with intravenous fluid administration to maintain renal perfusion.

A dabigatran (Pradaxa) patient's death from a brain hemorrhage following a fall has again highlighted concerns over lack of an effective reversal agent for the drug.

Despite treatment with recombinant factor VII, the 83-year-old man deteriorated rapidly as the bleeding spread across most of the left hemisphere of his brain in just six hours, according to Richard H. Schmidt, MD, PhD, of the University of Utah in Salt Lake City, and colleagues./.../

Although I've done a hundred or so public talks around the world in the last few years around my book The Great Disruption, speaking at the opening session of the annual TED event is an experience and opportunity like no other. Another speaker backstage, feeling similarly hyped about the opportunity, described it to me as being like "the world series of public speaking"!

For me the pressure was really on because I was going into the lion's den of "techno optimists" -- those who believe that technology can solve everything. My message is a tough one for this audience -- that sure technology will do wonderful things for us, but the reality is we are going to face some very difficult consequences of our overloading of planet earth, and its too late now to stop those consequences. I argue strongly that humans are amazingly capable and will recover from this inevitable crisis and indeed in the end build a stronger and happier society.

My focus in life is to motivate and inspire people to act on the urgent challenges of climate and sustainability and it is a real honor to speak to the TED crowd. This passionate and engaged community has an amazing capacity to make a difference in the world with their creativity, influence, innovation and entrepreneurship so I was delighted to make a contribution in the opening session./.../

WASHINGTON, Feb 29 (Reuters) - The World Bank said on Wednesday developing countries appear to have already met a United Nations goal to halve extreme poverty in the world's poorest countries by 2015.

The Washington-based development lender said preliminary data showed developing countries as a group reached the goal - the first of eight U.N. Millennium Development Goals - in 2010.

The MDGs are a set of targets adopted by world leaders at the United Nations in 2000 to fight poverty, hunger and disease in poor countries.

"We are now confident that the developing world as a whole has reached the first of the Millennium Goals and reached that goal in 2010 despite the crisis," said Martin Ravallion, director of the World Bank's research group and lead author of the report./.../

DIA DA MÚSICA CLÁSSICA

Uma homenagem ao nascimento de Heitor Villa-Lobos.

Destaca-se por ter sido o principal responsável pela descoberta de uma linguagem peculiarmente brasileira em música, sendo considerado o maior expoente da música moderna no Brasil, compondo obras que enaltecem o espírito nacionalista onde incorpora elementos das canções folclóricas, populares e indígenas.

É possível encontrar na obra de Villa-Lobos preferências por alguns recursos estilísticos: combinações inusitadas de instrumentos, arcadas bem puxadas nas cordas, uso de percussão popular e imitação do cantos de pássaros. O maestro não defendeu nem se enquadrou em nenhum movimento, e continuou por muito tempo desconhecido do público no Brasil e atacado pelos críticos. Também se encontra em sua obra uma forte presença de referências a temas do folclore brasileiro.

Como educador, sua idéia não era ensinar música nas escolas e sim despertar o interesse dos jovens pela música, visando ampliar as platéias dos concertos de música clássica no Brasil.

A data foi instituída em 13 de janeiro de 2009 no Governo Lula.

O público de música clássica no Brasil cresce a cada dia. Em 6 anos, o público da Orquestra Sinfônica do Estado de São Paulo subiu de 68.134 para 172.879 espectadores.


O Brasil realiza hoje em torno de 60 festivais por ano de música clássica, que vem batendo recordes de público a cada edição. O maior deles, o Festival Internacional de Inverno de Campos de Jordão mantem um público anual em torno de 80 mil pessoas.
